Home care provider Abbots Care, which covers Hemel Hempstead, is preparing to welcome new service users and staff after being awarded a reablement contract by Hertfordshire County Council.
Under the contract, Abbots Care reablement services, previously known as Specialist Care at Home, will play a part in the integration between health and social care systems in Hertfordshire by supporting hospital discharges, preventing admission to hospital from the community and partnering with health and community services.
